McIntosh Trail Family Practice’s Referral Department

Needing a referral to a specialist? We can help.

McIntosh Trail Family Practice is an independent medical practice that accepts most major insurance plans from HMO’s to PPO’s. Some of these major insurance companies may require you to have a referral from your PCP in order to see the specialist. This is where our referral department can assist in getting that done for you.

Mrs. Christy Cannafax, our referral coordinator has been with our practice for 5 years.  She has over 14 years experience in the medical field.  She enjoys and is dedicated to working with our patients to make sure all their referral needs and questions are met. Christy handles referrals for all three of our locations; Griffin, Stockbridge, and Heron Bay.

The hours of operation for our referral department are Monday through Friday, from 8:30 to 2:30. The direct number for referrals is 770-228-5407 ext. 162. We do ask that you try to give a 24 hour notice for any referral request that you have.

When leaving a message for a request please be sure to leave the following information:

  • Name
  • DOB
  • Telephone #
  • Type of specialist you are requesting and their name and phone #
  • Appointment date and location (some providers have multiple locations)

We will work closely with our patients to ensure that all specialty needs are met and suit the patients requirements/medical needs for their condition.

When referring patients to a hospital we use Spalding Regional, Southern Regional, Henry Medical Center, and Fayette Piedmont. Our Kaiser patients have the choice of Henry Medical Center, Piedmont, or Northside Hospital.

There can be delays in getting a referral if:

  • The referral request was not generated through your previous visit. In this situation, your request would have to be forwarded to the provider last seen for approval.
  • If you have never been seen by our office and are not an established patient. This is usually for patients that have an HMO policy. We must have the clinical information in your chart supporting the reason for referring you out to a specialist.

All referrals are subject to the restrictions/conditions that may be enforced by your insurance company.

Once your referral is complete, it will then be forwarded to the specialist office.

Please keep in mind that some insurance companies can take anywhere from 48 to 72 hours to approve/process a referral. Though this can be very frustrating, please know that our staff is working diligently in getting your referral processed as quickly as possible.
